How much should I boost?+

Subtle is usually better. 2-4 dB makes an audible difference. Past 6 dB you start running into clipping or muddy sound.

What frequency ranges are each band?+

Bass: under 200 Hz. Mids: 200-3000 Hz (where vocals sit). Treble: above 3000 Hz.
