Instant and active dry are roughly 1:1 by weight. Fresh yeast is about 3× the weight (since it’s mostly water). The traditional ratio is one packet (7g) of dry yeast = 21g (0.6 oz) of fresh.

For most home recipes, yes. Instant doesn't need proofing in warm water first, which is the only practical difference at small scales.
